CCBQ’s Care Coordination Programs: Queens Care Coordination, Non-Medicaid Care Coordination, Children’s Care Coordination and Pathway Home recently hosted Thanksgiving Turkey Distributions to residents in need in Brooklyn and Queens. The programs provided 75 clients with turkeys and fixings in insulated, reusable CCBQ bags, along with a reusable lunch bag/backpack containing personal protective equipment (PPE), including gloves, masks and hand sanitizer. Our staff’s time, energy and desire to provide a special moment in our client’s lives, amidst the tough times we are living in, brought joy to everyone involved. Our program and guests expressed how meaningful this event was to them as one client expressed how her Care Coordinator is the only stable person in her life. She noted how therapists and doctors always come and go, but her designated Care Coordinator has been a constant support and her rock who she cannot imagine not having in her life. Staff left feeling thankful and happy to make a difference in the lives of our clients.
Happy Thanksgiving and stay safe!
