On May 20th, 2019, Joan Sass, Clinical Director of Queens Transitional Team at Catholic Charities Brooklyn and Queens was cordially invited to attend the 2019 Heritage Provider Network Healthcare Innovation Awards for her work in the Catholic Charities Pathway Home Program. This innovative program under Integrated Health and Wellness Care Coordination helps individuals transition from long-term inpatient psychiatric hospitalizations to stable lives within the community.
Held at the New York Athletic Club in Manhattan, winners were announced in five categories at a luncheon ceremony. Mark Graham, Vice President of Program Services of Coordinated Behavioral Care (CBC) won the Heritage Innovation in Healthcare Delivery Award, which recognizes an innovator in the development of new modes of diagnosis, treatment and care who actively improves access to services and improves the quality of healthcare overall. Since joining CBC in 2014, Mark has transformed how New Yorkers with serious mental illnesses are integrated back into the community, one of CBC’s array of innovative programs.
