Brooklyn Health Care Coordination (BHCC)

Catholic Charities provides individuals with serious mental illness, complex medical needs and substance abuse issues with comprehensive care coordination and case management services. Our BHCC team works with the New York City Department of Health and Mental Hygiene and New York State Health Homes Program to help individuals meet their goals and continue living in the community. By helping them gain access to services tailored to their individual needs, we help people maintain a baseline level of functioning and sense of personal satisfaction.
