Case Management for Homebound Seniors

This program provides case management to homebound older adults, so that they can remain safely in their homes. Services include assessment, creation of comprehensive care plans, benefits/entitlements counseling, linkage to home-delivered meals and home care, counseling, information and referrals, and caregiver support. The program addresses all issues associated with advanced age.
