Home and Community Based Services (HCBS) and Community Oriented Recovery and Empowerment (CORE) provide opportunities for Medicaid beneficiaries with mental illness and/or substance use disorders to receive services in their own home or communities. HCBS currently offers employment and educational support and CORE offers psychosocial rehabilitation support and peer empowerment support. Our services keep individuals out of the hospitals; help individuals stay well-connected to a safety net of community-based resources and supports; and follow a recovery model of care that emphasizes and supports a person’s potential for recovery.
